How to Schedule
Our records indicate that you're due for your cancer-related follow-up care appointment.
We follow national guidelines that recommend that all long-term survivors of childhood cancer return each year for a follow-up visit. These visits help us to partner with you to maintain your health and to manage or monitor for late effects (health problems that may have resulted from your treatment).
If you are unsure of where to schedule your appointment, call the clinic where you last visited for cancer-related follow-up care.
Here are the numbers to call to schedule:
CHOP General Survivorship Clinic: call 215-590-3025, option 1 to reach the cancer center front desk and ask to schedule a survivorship visit.
CHOP Brain Tumor Survivorship Clinic: call the survivorship clinic at 215-590-0432 (Claire) and ask to schedule a survivorship visit for survivors of brain tumor.
