How to Schedule

Our records indicate that you’re due for your cancer-related long-term follow-up care appointment.  

We follow national guidelines that recommend that all long-term survivors of childhood cancer return each year for a follow-up visit. These visits help us to partner with you to maintain your health and to manage or monitor for late effects (health problems that may have resulted from your treatment).  

If you are unsure who or where to call, or are running into difficulties, try contacting the last clinic you went to for support. They can help you navigate where to go.

Here are the numbers to call to schedule:

CHOP General Survivorship Clinic: call 215-590-3025, option 1 to reach the cancer center front desk and ask to schedule a survivorship visit.  

CHOP Brain Tumor Survivorship Clinic: call the survivorship clinic at 215-590-0432 (Claire) and ask to schedule a survivorship visit for survivors of brain tumor.

Penn Survivorship Program: contact Donna (call 215-615-3329, text 267-324-4540, or email donna.pucci@pennmedicine.upenn.edu). Include your name and date of birth and ask to schedule an appointment in the survivorship program.

Nationwide Children’s Bone Marrow Transplant Long-Term Follow-Up Clinic: call 614-722-8977 to schedule an appointment if you received a bone marrow transplant as part of your cancer treatment.
